Prescriptions are only considered after a documented consultation with an Osteopathic Midlife Health provider.
During your consultation, your provider may review your symptoms, health history, goals, risk factors, lifestyle, medications, allergies, relevant lab work, and prior medical information. Medication is only prescribed when clinically appropriate and legally permitted.
Prescription medication is not guaranteed. When medication is prescribed, it is used as part of a broader care plan that may also include nutrition guidance, movement, sleep support, stress management, hormone and metabolic care, lifestyle strategies, follow-up visits, and other personalized therapies or practices.
You agree to use medications only as directed and to promptly report side effects, changes in health, new medications, or concerns.
Your provider may recommend lab testing, outside medical records, specialist evaluation, primary care follow-up, imaging, or in-person care before or during treatment.
You are responsible for completing requested labs or providing requested records when needed for safe and appropriate care. Failure to complete recommended testing or follow-up may limit our ability to prescribe medication, continue treatment, or provide certain services.
Osteopathic Midlife Health does not replace your primary care provider, gynecologist, emergency provider, mental health provider, or other specialists. You are encouraged to maintain appropriate routine and preventive care.
Appointments must be scheduled through our approved scheduling process.
If you need to cancel or reschedule, please do so as early as possible. Osteopathic Midlife Health may charge cancellation, late cancellation, or no-show fees according to the policy provided at the time of booking or in your patient documents.
We reserve the right to reschedule appointments due to emergencies, provider availability, technology issues, or other circumstances.
You agree to pay all applicable fees for services, consultations, programs, memberships, labs, products, prescriptions, or other offerings as disclosed at the time of purchase or booking.
Payment may be required before or at the time services are provided. Fees may vary by service and may change over time.
Unless otherwise stated in writing, payments for completed consultations, medical visits, digital services, and administrative services are non-refundable. Refund eligibility for packages, memberships, programs, or prepaid services will be governed by the specific terms provided at purchase.
You are responsible for any charges from pharmacies, labs, third-party vendors, payment processors, or outside providers.
Osteopathic Midlife Health may or may not participate with insurance plans. If we do not bill insurance directly, you are responsible for payment at the time of service.
If superbills or documentation are provided, reimbursement is not guaranteed and depends on your insurance plan. You are responsible for understanding your insurance benefits, coverage, deductibles, and out-of-network policies.

We reserve the right to discontinue care for abusive behavior, repeated noncompliance, unsafe use of medications, failure to complete required follow-up, nonpayment, or other reasons permitted by law and professional standards.
